I am bleeding heavy and clotting. is it a misscarriage and do i need a DNC?

i have been spotting for a month then suddenly i am having heavy bleeding with clots. I went to the doctor and she advised i dont need a scanning and just asked for a blood test. i dont know the result yet. I dont even know if it was a miscarriage as i have always had irregular periods. I want to know if i need a DNC if it was a miscarriage. Please help.i need answers. i am in a place were there are no Gynec.
